What data and why

Through the “Ask for an appointment” form we collect your name, phone number, optionally your e-mail and your message — solely to reply and book your visit (Art. 6(1)(a) and (b) GDPR). Please don’t include detailed health information in the form.

How long

We keep form messages for up to 12 months, unless they become part of medical records, which we keep as required by law.

Your rights

You have the right to access, rectify, erase, restrict and port your data, to withdraw consent, and to lodge a complaint with the Polish data protection authority (UODO).
